TEST COMPLETION & SUBMISSION DETAILS: To earn 1 Continuing Education Unit (CEU), the issue date (August 2014) must fall within your certificate/ credentialing period. You are only allowed to submit 3 passing tests per year. If SNS Credentialed: You have no limit on the number of tests that can be submitted within your 3-year period. To pay by check: Mail this completed form before your expiration date. Include $10 (SNA Members) or $16 (non-members) for processing to: SNA, Attn: PDA, PO Box 759297, Baltimore, MD 21275-9297. Do not send cash! To pay with a credit card: Pay for and take the test online at www.schoolnutrition.org/OnlinePDAs. Processing fees for tests completed online are $8 (SNA Members) or $14 (non-members). Due to administrative costs, refunds will not be made for any reason. Certificate/ Credentialing FAQs Q: Will renewal notices be sent out to individuals who earn their certificate? A: Yes. Three months before your certificate is scheduled to expire, SNA will send you a letter and a renewal form. To renew your certificate, simply submit the form with payment to SNA at least 4 to 6 weeks prior to the expiration date. Q: How will I know whether I am picked for an audit during certificate renewal? A: SNA uses an automated, random selection process for audits. Each month, SNA will identify a percentage of individuals for auditing during their certificate renewal. If you are selected for an audit, notification to send CEU documentation to SNA will be included in your annual renewal notice. Q: I passed the SNS exam. How long is it until the credentialing renewal period? A: The credentialing renewal period is every 3 years. Q: Is the National Restaurant Association an approved provider of Key Area programs? A: No, the National Restaurant Association (NRA) is not an approved provider, but SNA does accept NRA’s ServSafe® as a core course.
